At the age of 19, Vitor Belfort relocated to the USA to compete. His first official mixed martial arts contest took place in Hawaii at a gathering named “Superbrawl.” He engaged in combat with Jon Hess. Jon was knocked out in 12 seconds by him. He later relocated to compete in the UFC, where he earned the moniker “The Phenom.” He defeated two opponents to win the UFC 12 Heavyweight Tournament during his UFC debut event. Belfort, who won at the age of 19, made history by being the youngest combatant to triumph inside the octagon. In his subsequent fight, he defeated Tank Abbott, the UFC 6 runner-up, in a non-tournament battle by knocking him down and applying ground and pound to finish the fight.

In 1997, he faced off against American Greco-Roman wrestler Randy Couture, but lost. Then he engaged in combat with Joe Charles, which he ultimately won via armbar. He faced Alistair Overeem in the opening round of the 2005 Middleweight Grand Prix, which he lost via first-round guillotine choke. At UFC 33, the first UFC event to be held in Las Vegas, he was supposed to face Tito Ortiz in the main event. Later, because of his injuries, the fight was called off. On January 31, 2004, he fought again, this time against Randy Couture for the UFC Light Heavyweight Championship. In just 49 seconds, he defeated Randy Couture. On August 21, 2004, they engaged in their third match, which Couture won by doctor stoppage in the third round to reclaim the title. After that, on February 5, 2005, he fought and lost to Tito Ortiz.

In a rematch with Alistair Overeem, he also competed in Strikeforce, where Belfort lost by unanimous decision. He was defeated by Pride Welterweight Champion Dan Henderson in a unanimous decision at Pride 32: The Real Deal on October 21, 2006. He tested positive for the banned drug 4-hdroxytestosterone after the fight. Belfort claimed in his defense that he had bought an over-the-counter supplement containing 4-Hydroxytestosterone. Belfort added that after having surgery to repair a damaged meniscus in his knee in the summer of 2006, Brazilian endocrinologist Dr. Rodrigo M. Greco administered him rehabilitative injections that may have resulted in him receiving 4-hydroxytestosterone.

Dr. Greco eventually provided a declaration to the Nevada State Athletic Commission admitting that he had indeed administered testosterone post-surgery to Belfort. Belfort might not have been aware of the testosterone, but the NSAC emphasized that even if he received injections from a doctor who did not disclose that they contained anabolic steroids, it would still be against the rules to use them. On December 21st, 2006, he received a $10,000 fine and a nine-month suspension starting from the hearing date. At Cage Rage 21 in England on April 21, 2007, he defeated Ivan Serati through technical knockout.

Similar to that, he won against James Zikic at Cage Rage 23 on September 22, 2007. He received a unanimous decision victory and the Cage Rage light heavyweight belt. He briefly trained with Chute Boxe in Brazil before his Affliction match against Terry Martin. Dana White said at a press conference that Belfort had been re-signed by the UFC. He then knocked out Yoshihiro Akiyama in one minute and 52 seconds to win the fight. He was chosen to mentor Wanderlei Silva in The Ultimate Fighter: Brazil.

The rematch between Silva and Belfort was scheduled to take place at UFC 147 on June 23, 2012. Belfort was forced to withdraw from the fight on May 27, 2012, citing a broken hand sustained while training, and Rich Franklin took his place. He competed against Dan Henderson at light heavyweight at UFC Fight Night 32 on November 9, 2013, and won. On May 23, 2015, at UFC 187, Chris Weidman defeated him by way of technical knockout. On November 7, 2015, at UFC Fight Night 77, he faced and defeated Dan Henderson in a rematch.
